Do not take methylene blue with SSRIs, SNRIs, MAOIs, or other serotonergic medications. This combination can cause serotonin syndrome, a medical emergency.

Methylene blue carries an FDA boxed warning, the agency's highest safety alert: combined with SSRIs, SNRIs, MAOIs, opioids, or other serotonergic medications, it can cause serious or fatal serotonin syndrome. It can cause severe hemolysis in people with G6PD deficiency, a contraindication rather than just a caution. Other effects include blue-green discoloration of urine and sometimes skin, nausea, headache, and dizziness. Dose matters significantly: higher doses reverse the effect seen at low doses, shifting the compound from antioxidant to pro-oxidant.
