This Privacy Policy describes how euVita Health, Inc. (“euVita Health,” “Company,” “we,” “us,” or “our”) and, where applicable, euVita Medical Group PLLC (“Medical Group”) collect, use, disclose, maintain, and protect personal information obtained through our website, online forms, lead intake forms, telehealth-related forms, SMS/text message programs, email communications, artificial intelligence or automated communication tools, and other online or offline interactions with individuals.

This Privacy Policy applies to information collected through the website located at euvitahealth.com, including any related pages, forms, landing pages, communication tools, and digital services operated by or on behalf of euVita Health, Inc. and/or euVita Medical Group PLLC.

euVita Health, Inc. is the management services organization associated with the website, administrative services, business operations, marketing communications, technology systems, and related non-clinical support functions. euVita Medical Group PLLC is the professional medical group associated with certain clinical and telehealth services.

To the extent any information constitutes protected health information under applicable healthcare privacy laws, such information is also governed by the applicable Notice of Privacy Practices issued by euVita Medical Group PLLC. This Privacy Policy is intended to supplement, and not replace, any applicable Notice of Privacy Practices.

By accessing or using our website, submitting information through our forms, communicating with us, opting into communications, or otherwise interacting with us, you acknowledge that you have read and understood this Privacy Policy.

Information relating to products or services requested, services inquired about, appointment requests, payment-related information, billing information, purchase history, transaction history, subscription information, service preferences, and related commercial interactions.

If you opt in to receive SMS/text messages: your mobile telephone number, consent status, opt-in date and time, opt-in source, form version, disclosure language shown at the time of opt-in, IP address where available, message history, STOP requests, HELP requests, START requests, and other SMS compliance records.

If you opt in to receive email marketing communications: your email address, consent status, subscription preferences, email engagement information, unsubscribe requests, and related communication records.

H. Audio, Voice, and Similar Information

Where permitted by law, voice-related information, voicemail messages, call recordings, call transcripts, artificial voice interactions, AI-assisted communication records, or similar communication data. Where the law requires consent or notice before recording a call, we will provide such notice or obtain such consent.

I. Geolocation Information

Approximate location information derived from your IP address or information you voluntarily provide, such as your mailing address or state of residence.

J. Inferences and Preferences

Inferences regarding your preferences, interests, service needs, communication choices, form activity, and interaction history based on the information collected.

2. Sources of Personal Information

We may collect personal information from the following sources:

Certain information collected, created, received, maintained, or transmitted by euVita Medical Group PLLC may constitute protected health information under applicable healthcare privacy laws. Where information is protected health information, euVita Medical Group PLLC will use and disclose such information in accordance with its Notice of Privacy Practices and applicable law. The Notice of Privacy Practices explains how protected health information may be used and disclosed, your rights regarding such information, and the legal obligations applicable to the Medical Group. This Privacy Policy applies to personal information generally and should be read together with the applicable Notice of Privacy Practices where medical or healthcare information is involved.

5. Consumer Health Data

Some health-related information collected through the website, intake forms, communication tools, marketing forms, or non-clinical services may be considered consumer health data under certain state privacy laws, even if such information is not protected health information under healthcare privacy laws. Consumer health data may include information relating to health conditions, symptoms, treatments, medications, services requested, wellness goals, telehealth inquiries, or other information reasonably linkable to a consumer’s physical or mental health status. Consumer health data is handled in accordance with our Consumer Health Data Privacy Policy, which is linked from our website homepage, and applicable state privacy laws.

6. SMS/Text Message Privacy

If you provide your mobile number and opt in to receive marketing SMS/text messages, we may use your mobile number to send recurring marketing and promotional text messages from euVita Health, Inc. We may collect and maintain SMS-related information, including your mobile number, opt-in status, date and time of consent, consent source, campaign source, message history, STOP requests, HELP requests, START requests, and related compliance records.

Consent to receive marketing SMS/text messages is not a condition of purchase, treatment, payment, enrollment, eligibility, or receiving healthcare services.

We do not sell your mobile telephone number or SMS consent. We do not share your mobile telephone number or SMS consent with unaffiliated third parties for those third parties’ own independent marketing purposes.

Text messaging originator opt-in data and consent will not be shared with any third parties, excluding aggregators and providers of the text message services.

You may opt out of marketing SMS/text messages at any time by replying STOP to any marketing text message or by contacting us through any other reasonable method. We will honor revocation requests within ten (10) business days of receipt, or any shorter period required by applicable law. 7. Email Marketing Privacy

If you provide your email address and consent to receive marketing emails, we may send you marketing and promotional emails relating to euVita services, products, updates, resources, or offerings. You may unsubscribe from marketing emails at any time by clicking the unsubscribe link included in the email or by contacting us directly; unsubscribe requests are processed within ten (10) business days. Opting out of marketing emails will not prevent us from sending transactional, administrative, service-related, healthcare-related, or legally required communications.

8. Artificial Intelligence, Automated Voice, and Communication Technologies

euVita may use artificial intelligence, automated systems, prerecorded voice, artificial voice, voicemail technology, call-assist technology, chat tools, or similar communication technologies in certain outbound or inbound communications. These technologies may be used for administrative communications, customer support, service reminders, form-completion reminders, marketing communications, voicemail messages, call routing, documentation support, or other communications permitted by law.

Where required by law, euVita will obtain prior consent before using artificial voice, prerecorded voice, AI-generated voice, or automated calling technology to contact you, and any such call will identify the use of an artificial or AI-generated voice where required by law.

You may opt out of AI-generated, artificial voice, prerecorded voice, or automated marketing communications at any time by contacting us or by following the opt-out instructions provided in the applicable communication. euVita will not use AI voice or automated communication technology to override a consumer’s opt-out request, do-not-call request, STOP request, unsubscribe request, or any other reasonable revocation of consent.

9. Cookies and Tracking Technologies

We may use cookies, pixels, web beacons, tags, analytics tools, and similar tracking technologies to operate the website, remember preferences, improve functionality, measure performance, analyze traffic, understand user interactions, and improve our services. The information collected through these technologies may include IP address, browser type, device type, operating system, referring pages, pages visited, time spent on pages, links clicked, form interactions, and similar information.

We use cookies, pixels, tags, and similar technologies — including the Meta Pixel — on our website for analytics and for our own marketing and advertising, including retargeting. We do not sell your personal information. We limit the data shared with these advertising providers and apply privacy safeguards, including Meta’s Limited Data Use setting, on pages where health-related information may be present, and we do not provide health condition details to advertising providers. You can manage or withdraw your consent to advertising and analytics technologies at any time through the cookie preferences on our website.

You may be able to adjust cookie settings through your browser. If you disable cookies, certain website features may not function properly. Where required by applicable law, we will provide additional cookie notices, consent tools, or opt-out mechanisms.

10. How We Disclose Information

A. Affiliated Entities

We may disclose information to euVita Medical Group PLLC, other entities operating under the euVita brand, or affiliated administrative, clinical, or operational entities, where permitted by law and consistent with the purpose for which the information was collected.

B. Healthcare Providers and Medical Personnel

Where applicable, we may disclose information to licensed healthcare providers, clinical staff, telehealth providers, pharmacies, laboratories, care coordinators, or other parties involved in healthcare services or related administrative support.

We may disclose information to vendors and service providers that perform services on our behalf under contractual confidentiality and data protection obligations, including website hosting, CRM management, HubSpot implementation, SMS messaging, email delivery, analytics, scheduling, payment processing, telehealth technology, customer support, compliance, cybersecurity, cloud storage, and professional services.

D. Legal, Regulatory, and Compliance Purposes

We may disclose information where required or permitted by law, regulation, subpoena, court order, legal process, governmental request, regulatory inquiry, audit, investigation, or professional obligation.

11. No Sale or Sharing of Personal Information

We do not sell your personal information for money or other valuable consideration. We use certain limited information — primarily online activity information — for our own marketing and advertising, including retargeting through advertising partners such as Meta. Under some state privacy laws, this use may be considered “sharing” for cross-context behavioral advertising. You can control or withdraw it at any time through the cookie preferences on our website. Text messaging originator opt-in data and consent will not be shared with any third parties, excluding aggregators and providers of the text message services. We do not sell or share your SMS opt-in consent or mobile telephone number with unaffiliated third parties for their own marketing purposes.

12. Data Retention

We retain personal information for as long as reasonably necessary to fulfill the purposes described in this Privacy Policy, provide services, maintain business records, comply with legal obligations, resolve disputes, enforce agreements, manage communication preferences, maintain consent records, and support compliance obligations. Consent records, SMS opt-in records, email opt-in records, opt-out records, unsubscribe records, do-not-call records, suppression records, and related compliance records may be retained for a period reasonably necessary to demonstrate compliance with applicable law. When personal information is no longer needed for these purposes, we delete, de-identify, or securely archive it.

15. Marketing Opt-Outs; Do Not Call and Suppression Requests

You may opt out of marketing SMS/text messages by replying STOP to any marketing text message, opt out of marketing emails by clicking the unsubscribe link in the applicable email, or contact us directly to request removal from marketing communications or placement on our internal do-not-call and suppression lists. We will honor opt-out and suppression requests within ten (10) business days, or any shorter period required by applicable law. Even if you opt out of marketing communications, we may still send transactional, administrative, service-related, healthcare-related, account-related, or legally required communications.

16. California Privacy Rights

California residents may have additional rights under California privacy laws, including the right to know, access, correct, delete, limit certain uses of sensitive personal information, and opt out of the sale or sharing of personal information, subject to applicable exceptions.
