Consumer Health Data Privacy Policy

euVita Health, Inc.Effective June 12, 2026

This Consumer Health Data Privacy Policy (“Policy”) explains how euVita Health, Inc. (“euVita Health,” “we,” “us,” or “our”) and, where applicable, euVita Medical Group PLLC (“Medical Group”) collect, use, disclose, retain, and protect consumer health data submitted through our website, intake forms, telehealth-related forms, communication tools, and related digital services. A link to this Policy is available from the homepage of our website.

This Policy applies to consumer health data subject to applicable state consumer health privacy laws, including the Washington My Health My Data Act and similar laws in other states. To the extent any information constitutes protected health information under HIPAA or other healthcare privacy laws, such information is instead handled in accordance with the Notice of Privacy Practices issued by euVita Medical Group PLLC.

1. Consumer Health Data We May Collect

Depending on how you interact with us, we may collect consumer health data, including:

  • Information about health conditions, symptoms, treatment interests, wellness goals, or healthcare needs.
  • Information submitted through intake forms, questionnaires, telehealth forms, or service request forms.
  • Information regarding medications, prescriptions, allergies, medical history, or treatment preferences.
  • Information about services requested, considered, scheduled, or received.
  • Communications concerning health, wellness, telehealth, medical services, or related care coordination.
  • Information related to appointment requests, provider communications, or administrative support.
  • Any other information that identifies your past, present, or future physical or mental health status, or from which such status may reasonably be inferred.

2. Sources of Consumer Health Data

We collect consumer health data directly from you, from website forms, from telehealth-related communications, from euVita-affiliated entities, from healthcare providers, and from service providers acting on our behalf. We do not purchase consumer health data from data brokers and we do not collect consumer health data from advertising networks.

3. Consent; How We Collect and Use Consumer Health Data

We collect and use consumer health data only (a) with your consent, or (b) to the extent necessary to provide a product or service that you have requested from us. Where consent is required, we will obtain it through a clear, affirmative act — such as your voluntary submission of an intake or service request form following clear disclosure — before collecting the data, and we will obtain separate consent for any collection or use that is not necessary to provide the product or service you requested.

Subject to the limits above, we may use consumer health data for the following purposes:

  • To receive, review, and process intake forms and service requests.
  • To provide, arrange, support, or coordinate telehealth and healthcare-related services.
  • To communicate with you regarding services, inquiries, appointments, care coordination, or administrative matters.
  • To operate, maintain, and secure our website, forms, communication tools, and services.
  • To maintain records, consent logs, communication preferences, and compliance documentation.
  • To send marketing communications, only where you have provided the required consent.
  • To comply with legal, regulatory, contractual, professional, and healthcare obligations.
  • To protect rights, safety, and security, including preventing fraud and unauthorized activity.

4. How We May Disclose Consumer Health Data

We disclose consumer health data only with your consent, as necessary to provide a product or service you have requested, or as otherwise permitted by applicable law, to the following categories of recipients:

  • euVita Medical Group PLLC and other euVita-affiliated entities.
  • Licensed healthcare providers, clinical personnel, care coordinators, pharmacies, laboratories, or healthcare support personnel, where applicable.
  • Service providers and processors bound by contract to process consumer health data only on our instructions, including providers of website hosting, CRM systems, telehealth technology, scheduling, SMS/text messaging, email services, customer support, payment processing, cybersecurity, compliance, and administrative operations.
  • Professional advisors, including legal, compliance, accounting, auditing, or consulting professionals.
  • Government authorities, regulators, courts, or law enforcement where required by law.
  • Other parties at your direction or with your consent.

We do not disclose consumer health data to third parties for those third parties’ own advertising or marketing purposes.

5. No Sale of Consumer Health Data

We do not sell consumer health data. We will not sell consumer health data without first obtaining your separate, valid authorization meeting the requirements of applicable law. We also do not sell mobile numbers, SMS consent records, or SMS opt-in information, and we do not share such information with unaffiliated third parties for those third parties’ own independent marketing purposes.

6. Your Consumer Health Data Rights

Depending on your state of residence and applicable law, you have the right to:

  • Confirm whether we collect, share, or sell your consumer health data.
  • Access your consumer health data, including a list of the third parties and affiliates with whom we have shared it and a means of contacting them.
  • Withdraw consent for our collection or sharing of your consumer health data.
  • Request deletion of your consumer health data, subject to applicable exceptions.
  • Not be discriminated against for exercising any of these rights.

To exercise these rights, contact us at support@euvitahealth.com or 1 (888) 554-0070. We will respond to a verified request without undue delay and in any event within forty-five (45) days of receipt. Where reasonably necessary, we may extend that period once by an additional forty-five (45) days after notifying you of the extension and the reason for it.

7. Right to Appeal

If we decline to act on your request, you may appeal our decision by contacting us at support@euvitahealth.com with the subject line “Consumer Health Data Appeal” within a reasonable time after our decision. We will respond to your appeal in writing within forty-five (45) days, including a written explanation of the reasons for our decision. If your appeal is denied, you may contact your state Attorney General to submit a complaint; for Washington residents, the Attorney General may be contacted at www.atg.wa.gov.

8. Request Verification

For privacy, security, and legal compliance purposes, we may take reasonable steps to verify your identity before responding to a consumer health data request. We may request information reasonably necessary to confirm that you are the person to whom the request relates or that you are authorized to act on that person’s behalf.

9. Data Retention

We retain consumer health data only for as long as reasonably necessary to fulfill the purposes for which it was collected, provide or support requested services, maintain legally required records, comply with legal obligations, resolve disputes, enforce agreements, and preserve consent and opt-out records. When consumer health data is no longer needed for these purposes, we delete or de-identify it.

10. Data Security

We restrict access to consumer health data to those employees, processors, and service providers who need access to further the purposes described in this Policy, and we use reasonable administrative, technical, and physical safeguards designed to protect consumer health data against unauthorized access, disclosure, alteration, loss, misuse, or destruction. However, no website, system, platform, or method of electronic transmission can be guaranteed to be completely secure.

11. Relationship to Other Policies

This Policy should be read together with our Privacy Policy, SMS/Text Message Terms of Service, Terms and Conditions, Telehealth Informed Consent, California Notice at Collection, and any applicable HIPAA Notice of Privacy Practices. If there is a conflict between this Policy and a legally required HIPAA Notice of Privacy Practices with respect to protected health information, the HIPAA Notice of Privacy Practices controls as to that information.

12. Changes to This Policy

We may update this Policy from time to time. Any changes will be posted with an updated effective date, and material changes affecting previously collected consumer health data will be made only after providing any notice or obtaining any consent required by applicable law.
